Moving away from mass swim starts could help. So could swim warm-ups.

I also read an article that discussed the possibility of wetsuits contributing to some of the swim deaths. The total body compression of the wetsuit might be creating circulation problems, in combination with the stresses of open water, mass starts, race-day adrenalin, etc.
